Fees: $0 fee for initial and renewal certification process.
Examination Frequency: One time
Exam Locations: Statewide and National
Experience Requirement: Have a high school diploma or equivalency or have greater than 4 years experience in dialysis as of October 14, 2008. Have successfully completed a CDPH approved hemodialysis training program. Have successfully passed a standardized test approved by the CDPH or the examination offered by a national commercially available certification program.
Average Time to Process Application: 4 weeks.
Renewal Period: Every 4 years.
